Workers Compensation

Fees for workers compensation patients are billed directly to the insurance company or to the employer.

If less than 24 hours notice is given when cancelling your appointment a late cancellation fee of $25 may be charged and payment of this fee is the responsibility of the patient.

What To Bring With You

In order to process your account please bring to your appointment

  • Written Doctors referral
  • Details of your employer
  • Details of your insurance company
  • Date of injury
  • Claim Number
  • Name and contact details of your case manager if known
  • Reports, X-rays, MRI’s, CT scans etc and any other relevant information

Providing us with the above details at the time of initial consultation allows the consultation fee to be billed to the insurer. If these details or notification of acceptance of the claim are not available it is the policy of the practice to bill the consultation fee to the patient. This may be reimbursed to you by the insurer if/when the claim is approved.
